Firearms Applications

Firearms Application forms can be accessed on-line 24 hours a day, seven days a week.

Below is a breakdown of the items required for Firearms Applicants. Click on the appropriate link to access instructions and/or the necessary form(s).

**The falsification of information on any of the applications for firearms permits is a violation of N.J.S.A. 2C:39-10c and is a crime of the third degree. Any falsification may result in criminal charges against you.**

If you need an:

  • Initial New Jersey Firearms ID Card
  • Initial Application for Handgun

You will need to:

  • Complete form: NJSP S.T.S. 033 Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit
  • Complete form: NJSP S.P. 66 Consent for Mental Health Records Search
  • Complete: Universal Fingerprint Form
  • Call Firearms Investigations Unit at 732-212-6800 or email at  firearms@seasideheightspd.com to make an appointment for review of documents.
  • Submit $5.00 in Money Order (ONLY) made payable to: Borough of Seaside Heights for the initial Firearms ID Card
  • Submit $2.00 in Money Order (ONLY) made payable to: Borough of Seaside Heights for each handgun permit (Note: Applicants can only make application for up to 3 handgun purchaser permits at a time)
  • Submit to fingerprinting at a local Morpho-Trak facility.

If you need an:

  • Application for Additional Handgun(s) (only if you already have Firearms ID Card)

You will need to:

  • Complete form: NJSP S.T.S. 033 Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit
  • Complete form: NJSP S.P. 66 Consent for Mental Health Records Search
  • Complete form SBI-212A* (click here for instructions)
  • Call Firearms Investigations Unit at 732-212-6800 or email at  firearms@seasideheightspd.com to make an appointment for review of documents.
  • Submit $2.00 in Money Order (ONLY) made payable to: Borough of Seaside Heights for each handgun permit (Note: Applicants can only make application for up to 3 handgun purchaser permits at a time)

Fingerprints are not required for this application.
